We agree that some of this information is personal in nature and is considered “sensitive” (as defined by law). We take the protection of this personal information very seriously and treat it with the highest level of security and confidentiality. We ensure that data is not collected for other purposes (e.g. advice, statistics, etc.)
If an individual chooses not to provide the above information, we will notify you of any possible consequences of non-disclosure. For example, withholding certain information about your health condition may limit our ability to work together to provide care or achieve desired outcomes.
